WHERE THE EVIDENCE STANDS

Semaglutide sits on the largest evidence base of any compound on this site, including cardiovascular outcome trials. The 2026 record is about use outside the trials: persistence rising to 60.9 % at one year, falling physical activity among users, and musculoskeletal signals in claims data. A Yale study in August 2026 proposed a revised account of how the class acts on hunger circuits.

On 30 April 2026 the FDA proposed excluding semaglutide from the 503B bulks list, which would close the outsourcing-facility compounding route opened during the shortage years. Reports cite more than 455 adverse-event reports tied to compounded semaglutide.

PAPER
GLP-1 drugs recruit hunger neurons to sustain fat loss, Yale study finds

Researchers at Yale report that GLP-1 therapies recruit agouti-related peptide neurons, long treated as drivers of hunger, to help maintain fat loss.

PAPER
What GLP-1 users do differently: activity, bone and persistence

Reports in 2026 describe falling physical activity among people on GLP-1 drugs, claims-data signals on bone and joint conditions with long-term use, and one-year persistence nearly doubling since 2021.

FDA proposes to end 503B compounding of semaglutide, tirzepatide and liraglutide

A 30 April 2026 proposal would exclude the three GLP-1 drugs from the 503B bulks list, closing the outsourcing-facility route that opened during the shortage years.

Orforglipron approved; the oral GLP-1 question

April 2026 brought approval of orforglipron, the first oral small-molecule GLP-1 receptor agonist. Oral options so far produce less weight reduction than leading injectables but are easier to make and take.
